Medical Record Analysis and Chronology

Medical records can be very complex, convoluted, and lengthy. Our medical consultants will review the medical records and the applicable standards of care for you. Furthermore, our network of expert bedside nurses and legal nurse consultants can frequently do all of this for you at a fraction of the cost of a medical doctor. If we determine that your case should merit additional expert analysis by a medical doctor, without hesitation we will locate an experienced and renowned practitioner in the field for you.   • Summarize, translate, and impartially interpret the record
  • Organize, tab, paginate, or bates stamp medical records upon request

Audit Trail Analysis

In complex medical litigation, the electronic audit trail can be as important as the medical record itself. While the chart reflects what is documented, the audit trail reveals when documentation was created, accessed, modified, or entered after the fact.


A thorough audit trail analysis can uncover late entries, back charting, alterations, inappropriate access, and inconsistencies in the documented timeline of care. These findings are often pivotal in cases involving documentation integrity, disputed timelines, informed consent issues, or allegations of delayed or inadequate intervention.


With clinical expertise and a deep understanding of electronic health record systems, a legal nurse consultant translates audit data into clear, defensible conclusions. Independent/Defense Medical Exams

Your client has the right to have an observer present during a physical or mental examination. We assist by coordinating, attending, and documenting medical exams, ensuring the integrity of the process and protecting your client’s interests. As your representative, we take detailed notes, observe carefully, and audio record when permitted. We will help prepare the client before the exam regarding the process and can help prevent the answering of excluded questioning or medically irrelevant circumstantial questions. We will strive to locate a legal nurse consultant within your client's geographic area for this process in order to minimize costs.  

Expert Witness and Professional Location Services

If you are in need of an expert witness outside of our scope of practice or expertise, usually for a small fee we can locate doctors, nurses, physical therapists, life care planners, and other allied health professionals with outstanding credentials for you. We will contact them, perform a preliminary screening on your behalf, and provide you with the resume or CV of experts likely to be a good fit for your case. 

Deposition, Interrogatory, and Trial Preparation/Analysis

We can help you prepare for interrogatories, deposition, trial, or alternative dispute resolution by helping to formulate the specific clinical questions that need to be asked. We can also review transcripts of depositions and interrogatory responses in order to help you identify any inconsistencies between the testimony and the medical record.    

Medical Research and Reporting

We conduct relevant literature research, provide detailed analysis, and prepare written reports at your request. Thorough, evidence-based research can significantly strengthen your claims or expose weaknesses in opposing arguments.

Upon request, we can interview clients, key witnesses, current health care providers, and experts alongside you or on your behalf. As nurses and industry professionals, we are uniquely equipped to understand complex medical terminology and translate it clearly while putting individuals at ease.
